Please review the following before your visit.
Appointments & Arrival
We recommend arriving 10 minutes before your scheduled time to complete any outstanding intake forms and settle in before your visit. Arriving late may require us to shorten your appointment to stay on schedule for the members after you.
Cancellations & Rescheduling
We understand plans change. We ask for at least 24 hours’ notice to reschedule or cancel. Changes made inside this window, or missed appointments, may be subject to a cancellation fee.
Telehealth & In-Person Visits
Many consultations and follow-ups can be completed via telehealth when appropriate. In-person visits are available for labs, IV therapy, injections, and hands-on care. Your provider will recommend the right format for each visit.
Privacy & Confidentiality
Your health information is protected under HIPAA and used only for treatment, payment, and healthcare operations. You may request restrictions on your records, or a copy of them, at any time — just ask your care team.
Informed Consent
All treatments, including injections, IV therapy, peptide therapy, and GLP-1 medications, require informed consent. Your provider will walk you through expected benefits, potential risks, and alternatives before any treatment begins — no guesswork, no pressure.
Customized Care
Your health has its own story. Please share your full medical history, current medications, allergies, and any concerns before your visit, so your provider can build the safest, most effective plan for you.
